PLANNING, HEALTH & PHYSICAL ACTIVITY SURVEY

Information by Design (IbyD) are undertaking this survey, on behalf of Active Derbyshire.  We would like to understand more about how health and physical activity are considered in the planning system in Derbyshire.  We are asking people from different professions to complete this survey, including colleagues from planning, leisure, transport and public health.

We would like you to give your honest appraisal of the situation and to contribute your professional opinion. This will help us to understand the current situation and to look at how we can make improvements to support the health and wellbeing of residents.

The information you give will only be used for the purposes of the survey, and will be anonymised so your individual responses will not be provided to Active Derbyshire.  Your individual responses will be combined with others to provide a report to the partnership.
