Thank you for agreeing to take part in this survey about your experience of pregnancy.  The survey asks some questions about your health, the way that you are feeling or felt during your pregnancy, and your experiences.  It should take around 15 minutes to answer the questions.  Please only answer the questions if you live in Stoke-on-Trent.

We will use the information you give us to help us to make sure that the right services are there for pregnant women in the future – so your views are really important.

• There are no right or wrong answers.  We are interested in your opinion.

• Please do not ask anybody else when you answer the questions. 

• Follow the instructions for each question carefully.  Most of the questions ask you to tick or enter information into a box.

• We will not pass your individual details on to anyone else but we will write a report about the results, so that we can help other people understand the things we ask you about.
