STOKE-ON-TRENT CITY COUNCIL - MIDWIFERY SURVEY 2017
Welcome to the Smoking in Pregnancy and Maternal Obesity survey, and thank you for your help with this research.
The survey asks some questions about your role, your experiences of working with pregnant women who smoke and/or are overweight or obese, and what you think could be done to improve the way women are supported in the future.
We will use the information you give us in this survey to provide a summary report to Stoke-on-Trent City Council who are responsible for Public Health functions. The Council will then work to ensure that you are given the right support to help reduce the level of smoking in pregnancy and maternal obesity in Stoke-on-Trent.
The information you provide is completely confidential and your individual responses will not be identifiable and will not be passed on to your line manager or any other party.
It should take around 10 minutes to answer the questions.
Only answer the questions if you work directly with pregnant women in Stoke-on-Trent.
There are no right or wrong answers. We are interested in your honest opinion.
Follow the instructions for each question carefully. Most of the questions ask you to tick one box or enter information into a box.
