STOKE-ON-TRENT CITY COUNCIL - MIDWIFERY SURVEY 2017
Welcome to the Smoking in Pregnancy and Maternal Obesity survey, and thank you for your help with this research.
The survey asks some questions about your role, your experiences of working with pregnant women who smoke and/or are overweight or obese, and what you think could be done to improve the way women are supported in the future.
We will use the information you give us in this survey to provide a summary report to Stoke-on-Trent City Council who are responsible for Public Health functions. The Council will then work to ensure that you are given the right support to help reduce the level of smoking in pregnancy and maternal obesity in Stoke-on-Trent.

The information you provide is completely confidential and your individual responses will not be identifiable and will not be passed on to your line manager or any other party.

Only answer the questions if you work directly with pregnant women in Stoke-on-Trent.

Paper copies of this survey have been delivered to the midwifery team across Stoke-on-Trent. If you have kindly completed a paper copy already, please note that you do not need to complete the online version as well.
